What Is the My GIST Circle Copay Card?
The My GIST Circle Copay Card can make it easier for you to pay for GLEEVEC®, no matter where you are in your KIT+ GIST treatment journey.
Here's how the card works:
- If you are starting on or currently taking any dosage of GLEEVEC, you can receive up to $25 per prescription refill for 20 refills. This is a total savings of up to $500 in copay assistance.
You may not receive the maximum copay assistance for each prescription. The total value of your copay card is determined by your specific copay.
Getting started with a copay card:
Step 1
If you already have a copay card, activate it here on this website. Or call 1-866-398-8750 to speak to an agent. If you don't have a card, you can use this same number to request one. You can also request a card on this website.
Step 2
After activating your copay card, present the card — along with your GLEEVEC prescription — at your retail pharmacy, specialty pharmacy, and/or mail-order pharmacy. You will then get valuable savings on your next 20 refill copayments for that prescription, as outlined above.
Who is eligible for the My GIST Circle Copay Card?
Patients who are being treated with GLEEVEC for KIT+ Gastrointestinal Stromal Tumor (KIT+ GIST) are eligible for the My GIST Circle Copay Card.
Patients who reside in the US and Puerto Rico, except for Massachusetts, and are not receiving government-related healthcare benefits are eligible for the My GIST Circle Copay Card.
Who is not eligible for the My GIST Circle/GLEEVEC Copay Card?
Anyone on a federal or state government-related healthcare program that pays in whole or in part for prescription drugs is not eligible for the My GIST Circle Copay Card. Examples of these programs are: Medicare, Transitional Assistance Program, Tricare, Medicaid, CHAMPUS, VA, and state maternal programs. Please note there are several more programs that qualify as government programs.
Patients who live in Massachusetts are not eligible for the My GIST Circle Copay Card.
Enrollment ends on 12/31/11. Card expires on 12/31/11.
