The My GIST Circle Copay Assistance Card FAQs:
1. What is the My GIST Circle Copay Assistance Card?
My GIST Circle Copay Assistance Card was created by Novartis Pharmaceuticals Corporation to help you save money on your out-of-pocket costs for GLEEVEC prescriptions. The program provides GLEEVEC patients being treated for KIT+ GIST with copay assistance.
If you are starting on or currently taking GLEEVEC, you can save up to $25 per prescription refill, for total savings of up to $500 of copay assistance. The My GIST Circle Copay Assistance program expires on 12/31/2013.
If patient is on a government program:
Unfortunately, anyone on a federal or state government–related healthcare program that pays in whole or in part for prescription drugs is not eligible for the My GIST Circle Copay Assistance Card. Examples of these programs are: Medicare, Transitional Assistance Program, Tricare, Medicaid, CHAMPUS, VA, and state maternal programs. Please note there are several more potential programs that qualify as government programs.
If patient is in Massachusetts:
Unfortunately, patients in Massachusetts are not eligible for the My GIST Circle Copay Assistance Card.
If outside the United States or Puerto Rico:
We regret that the My GIST Circle Copay Assistance Card is only available in the United States and Puerto Rico.
2. Who can get a My GIST Circle Copay Assistance Card?
The My GIST Circle Copay Assistance Card is only for patients taking GLEEVEC for KIT+ GIST.
3. How does the My GIST Circle Copay Assistance Card work?
As a qualified enrolled patient, you receive your activated card in the mail. Then when you go to the pharmacy, bring your My GIST Circle Copay Assistance Card with you, along with your GLEEVEC prescription from your doctor. Present the card to the pharmacist every time you fill your GLEEVEC prescription. If you are starting on or currently taking any dosage of GLEEVEC, you can save up to $25 per prescription refill, for total savings of up to $500.
4. How long is My GIST Circle Copay Assistance Card good for?
The enrollment period ends on 12/31/2013.
The program expires on 12/31/2013.
Novartis Pharmaceuticals Corporation reserves the right to rescind, revoke, or amend this program without notice.
5. Do I need a prescription?
Yes, you first need a valid signed GLEEVEC prescription from your doctor. The prescription and your My GIST Circle Copay Assistance Card must be presented at the pharmacy to receive the benefits of copay assistance.
6. Are there any insurance restrictions?
You may not utilize the copay assistance part of the program if you already receive your medications through federal or state healthcare programs. Examples of these programs are: Medicare, Transitional Assistance Program, Tricare, Medicaid, CHAMPUS, VA, and state maternal programs.
7. What if I don't have insurance at all; can I still use the My GIST Circle Copay Assistance Card?
Yes. You can still use the My GIST Circle Copay Assistance Card even if you don't have insurance.
8. Are all pharmacies participating?
All pharmacies are invited to process the My GIST Circle Copay Assistance Card for GLEEVEC prescriptions. There are no network requirements.
9. What else will participants receive from the My GIST Circle Copay Assistance Program?
Enrollees will receive an online copay assistance card to be printed at home, or will receive their copay assistance card via postal mail if preferred. At registration, you can also sign up for the My GIST Circle Support Program to receive regular email newsletters covering topics such as measuring treatment results and reducing the severity of some side effects, plus other useful, clinical, and relevant information.
10. I lost my card. How can I get a replacement?
If you have already received the My GIST Circle Copay Assistance Card and lost it, you may get a replacement card by calling 1-866-398-8750.
11. How do I use the My GIST Circle Copay Assistance Card if I use mail order or an online prescription program?
To redeem the benefits of the card, the following needs to be done:
- Mail in your prescription for GLEEVEC, along with a copy of the My GIST Circle Copay Assistance Card to the mail order pharmacy that your specific insurance covers.
- The card discount will be applied and you will be billed for the remaining amount, if any.
If your mail order pharmacy does not participate, you will be reimbursed with a Patient Mail-In Reimbursement Form, which we can send to you in the mail.
- When you receive your GLEEVEC medication from your mail order pharmacy (Caremark, for example), a receipt will be included.
- You will need to fill out the Patient Mail-in Reimbursement Form and send the form, the mail order pharmacy receipt, and a photocopy of your My GIST Copay Assistance Card to OPUS Health to receive reimbursement.
- Please note that each online/mail order pharmacy will have its individual policies and procedures for the patient to follow.
12. How do I get a My GIST Circle Copay Assistance Card?
If you are prescribed GLEEVEC and have been diagnosed with KIT+ GIST, you can enroll in the My GIST Circle Copay Assistance Card Program. If you qualify for a My GIST Circle Copay Assistance Card, you will receive an activated card in the mail. Some limitations may apply.
