What Is the My GIST Circle Copay Assistance Card?
The My GIST Circle Copay Assistance Card can make it easier for eligible patients to pay for GLEEVEC, no matter where you are in your KIT+ GIST treatment journey.
Here's how the card works:
- If you are starting on or currently taking any dosage of GLEEVEC, you can receive up to $25 per prescription refill, for total savings of up to $500 in copay assistance if you are eligible.
Getting started with a copay assistance card:
Step 1
If you already have a copay assistance card, activate it here on this website. Or call 1-866-398-8750 to speak to an agent. If you don't have a card, you can use this same number to request one if you are eligible. You can also request a card on this website.
Step 2
After activating your copay assistance card, present the card — along with your GLEEVEC prescription — at your retail pharmacy, specialty pharmacy, and/or mail order pharmacy. You will then get valuable savings on your copayments for that prescription, as outlined above.
Who is eligible for the My GIST Circle Copay Assistance Card?
Eligibility criteria include:
- Patients who are being treated with GLEEVEC for KIT+ GIST are eligible for the My GIST Circle Copay Assistance Card.
- Patients who reside in the United States and Puerto Rico, except for Massachusetts, and are not receiving government-related healthcare benefits are eligible for the My GIST Circle Copay Assistance Card.
Who is not eligible for the My GIST Circle/GLEEVEC Copay Card?
Anyone on a federal or state government-related healthcare program that pays in whole or in part for prescription drugs is not eligible for the My GIST Circle Copay Assistance Card. Examples of these programs are: Medicare, Transitional Assistance Program, Tricare, Medicaid, CHAMPUS, VA, and state maternal programs. Please note there are several more programs that qualify as government programs.
Patients who live in Massachusetts are not eligible for the My GIST Circle Copay Assistance Card.
Enrollment ends on 12/31/2013. Card expires on 12/31/2013.
